Support strategies for medication review and reconciliation

This learning activity will review evidence-based and patient-centered strategies to ensure accurate medication understanding.

At the conclusion of this training course the participant will be able to:

  1. Have increased knowledge regarding the definition of and application of medication reconciliation and medication review.
  2. Have greater competence related to gathering accurate medication information from the patient.
  3. Have greater competence related to working within scope of practice to minimize patient risk.
  4. Demonstrate greater confidence in their ability to accurately participate in the medication reconciliation and medication review process.

Successful completion includes:

  • Access the MI-CCSI Learning Management System.
  • Create a New User Profile (if a new learner to the system).
  • Participate in each desired learning activity.
  • Complete the course evaluation after each to obtain continuing education credits.

Audience

The course is intended for licensed and other healthcare professionals involved in the medication reconciliation and medication review process for care of patients in a clinic setting.

Faculty

Joseph Pomerville

Joe Pomerville is a Data Scientist for The Institute for Health & Business Insight (IHBI) at Central Michigan University. His work revolves around research, analysis, and the implementation of advanced analytics and data science techniques to make sense of and tell stories with data. Joe studied Geographic Information Science and Data Mining at Central Michigan University for both his undergraduate and graduate degrees.
