Medication Reconciliation, Safety & Medication Review

During this one-hour learning activity, the participant will explore medication safety, a cornerstone of high-quality patient care in primary care practices. The complexities of modern healthcare, coupled with the increasing number of medications prescribed to patients, have elevated the risk of errors and adverse events. This presentation will focus on three key areas: medication safety, medication reconciliation, and medication review. By understanding these processes and implementing best practices, primary care teams can ensure accurate medication management, reduce harm, and improve health outcomes for their patients.

At the conclusion of this training course the participant will be able to:

1. Enhance Awareness of Medication Safety Practices: Equip primary care providers with the knowledge and tools to identify and mitigate risks associated with medication errors to improve patient safety and outcomes.
2. Understand the Processes of Medication Reconciliation and Medication Review: Explore the step-by-step methods for conducting effective medication reconciliation and review to ensure accuracy, minimize discrepancies, and optimize therapeutic outcomes for patients.
3. Implement Actionable Strategies for Medication Management: Provide actionable recommendations and practical advice for integrating medication safety and reconciliation practices into day-to-day workflows in primary care settings.

Successful completion includes:

Faculty

Lynn Klima DNP, CNE, CPAFH, APRN, ANP-BC

Lynn is an adult nurse practitioner with over 40 years of experience spanning acute care, primary care, and community-based settings. She earned her Doctorate in Educational Leadership in 2018 from American Sentinel University and holds certifications as a Nurse Practitioner through the ANCC, a Nurse Educator through the NLN, and an Age-Friendly Health Care credential through IHI.

Her extensive career includes leadership roles focused on leadership development, performance improvement, care management delivery models, and care coordination initiatives to better support complex, chronically ill populations. Dr. Klima is passionate about fostering staff engagement in improving care delivery processes. Her doctoral research explored strategies to equip nurses in empowering patients with effective self-management support. As an implementation scientist, her key interests lie in integrating evidence-based practices that promote patient safety, transition management, patient-centered care, and leadership development across diverse care settings.

She currently serves as the Program Director for Mi-CCSI, an adjunct professor at The University of Olivet, and a preceptor for graduate and doctoral students at the University of Michigan.
