Intended Audience

These trainings are open to clinical practice teams who will be involved in serious illness conversations and management of palliative care services. This one day day session includes:

  1. Two hour session at the beginning of the day for the full clinical team that includes, but is not limited to:  The primary care provider, the Provider Delivered Care Manager (PDCM), and other clinical team members who will be involved in the serious illness conversations and management of palliative care services and care coordination.
  2. Full day session for the Provider Delivered Care Manager (PDCM) and other clinical team members who will be involved in the care coordination, completion of the biomedical and psychosocial assessments, and management and monitoring patient care based on the serious illness conversation.
