This session is intended for the full clinical team that includes, but is not limited to: The primary care provider, the Provider Delivered Care Manager (PDCM), and other clinical team members who will be involved in the serious illness conversations and management of palliative care services and care coordination.

10:15 am – 10:30 am | Break

The following sessions are intended for the Provider Delivered Care Manager (PDCM) and other clinical team members who will be involved in the care coordination, completion of the biomedical and psychosocial assessments, and management and monitoring patient care based on the serious illness conversation.
