The Collaborative Care Model (CoCM) Live Training Resources

During this multi-day live, virtual training course, the learner will gain knowledge and skills to implement a collaborative care model (CoCM) that includes a primary care provider, behavioral health care manager, and psychiatric consultant functioning as a team, using a shared care plan, to meet the needs of patients with mild to moderate behavioral health concerns.
