The Collaborative Care Model (CoCM) Demonstration Videos

Video 1: CoCM Referral to Behavioral Health Care Manager

This video demonstrates the process of the provider is referring the patient to the Behavioral Health Care Manager (BHCM) for assessment in the Collaborative Care Model (CoCM).


Video 2: CoCM Behavioral Health Care Manager Assessment

This video demonstrates the process of the BHCM is assessing the patient and beginning the Behavioral Activation process in the Collaborative Care Model (CoCM).


Video 3: CoCM Behavioral Health Care Manager Support (Behavioral Activation and Self-Management)

This video demonstrates the BHCM will be following up with the patient on Behavioral Activation goals and developing a self-management action plan in the Collaborative Care Model (CoCM).
