Elevating skills to support implementation of the CoCM care model

This nine part live webinar learning series will provide practical guidance to support the implementation of the Collaborative Care Model (CoCM) in primary care practices participating in the BCBSM CoCM initiative. Sessions will support skill development for the behavioral health care manager (BHCM) in motivational interviewing, problem solving treatment, and behavioral activation and assist the primary care team in developing processes and workflows to support implementation of the care model.

Additionally, three drop-in question and answer sessions for behavioral health care managers (BHCMs) will be offered after the January operations webinars on April 5th, April 27th & May 3rd. This is an opportunity to network with other BHCMs, ask questions and receive assistance and knowledge from experts and other BHCMs.

This session will explore the spirit of motivational interviewing and refine skills and knowledge in practical application.

Session 1: Problem Solving Treatment

This session will provide an overview of problem solving treatment and how it can be utilized to assist patients with developing problem solving skills utilizing self-management techniques.

Session 2: Behavioral Activation

This session will provide an overview of behavioral activation and how it can be utilized to treat patients with depression and/or anxiety.

Audience

These sessions are intended for behavioral health care managers (BHCM), practice leadership, and other clinical and non-clinical staff tasked with building the infrastructure and processes for implementing the Collaborative Care Management Model in the practice.  It is also applicable to nurses, social workers, advanced practice providers, medical assistants, community health workers and other care team members seeking to expand their knowledge and skills of motivational interviewing, problem solving treatment and behavioral activation.

Laura Saunders, MSSW

Laura Saunders is the Wisconsin State Project Manager for the Great Lakes Addiction, Mental Health, and Prevention Technology Transfer Centers. Since 2001, Laura has provided SBIRT and Motivational Interviewing training to physicians, nurses, medical students, psychologists, specialty addiction treatment providers, social workers, physical therapists, health educators, and staff who work in correctional settings. She has provided feedback and coaching to hundreds of social workers, correctional staff, and other human service providers who are interested in using evidence-based practices with fidelity. Laura joined the international group of Motivational Interviewing Network of Trainers (MINT) in 2006 (Sophia, Bulgaria) and is an active member of the Wisconsin MINT group.

Sue Vos, BSN, RN, CCM

Susan Vos is the Program Director at the Michigan Center for Clinical Systems Improvement. She has experience in working with multi-disciplinary teams to provide care for complex patients, extensive knowledge and expertise in practice transformation, developing and implementing care management curricula based on evidence-based guidelines, training of care manager and other clinical practice staff, implementing care models, and driving quality improvement strategies. She is the current president of the Case Management Society of America: Grand Rapids/Kalamazoo Chapter. Susan led the organizational team in the COMPASS (Care Of Mental, Physical And Substance-use Syndromes) Initiative funded by the Centers for Medicare & Medicaid Services (CMS) / Center for Medicare & Medicaid Innovation (CMMI), which evaluated the large-scale implementation of the TEAMcare model treating patients with depression and comorbid diabetes and/or cardiovascular disease. The initiative was implemented in 18 medical groups and 172 clinics across eight states.

Robin Schreur, BS, RN, CCM

Robin Schreur is a trainer for the Michigan Center for Clinical Systems Improvement (Mi-CCSI). She served as the Clinical Lead Care Manager in Ambulatory Care at Spectrum Health from 2012 until 2020. Her aim is to instill the importance of patient-centered care across the continuum of care. Robin has worked in a variety of settings including insurance-based and primary care practices, and as a behavioral health nurse at an inpatient facility. She has extensive experience in training and onboarding hundreds of clinicians in care management roles and motivational interviewing.
