Adolescent Collaborative Care Model Training
Behavioral health integration model for the adolescent population
The Adolescent CoCM Training is required for family and pediatric practices wanting to participate in the Adolescent CoCM program. This training will provide an overview of common Depression, Anxiety and other common co-morbid psychiatric disorders and treatment options for the adolescent population. Each practice is required to complete the Base CoCM training prior to attending this training.
This training was co-developed by the Michigan Center for Clinical Systems Improvement (Mi-CCSI), Michigan Collaborative Care Implementation Support Team (MCCIST), and Michigan Institute for Care Management Transformation (MICMT). Mi-CCSI ensures trainers are well versed in the concepts and have extensive experience to offer insight and application of the skills, tools, and information covered.
Mi-CCSI and MCCIST have been contracted by Blue Cross Blue Shield of Michigan as training entities for this project.
These trainings are open only to practices and their physician organizations (PO) participating in the BCBSM CoCM program.
DATES, TIMES & REGISTRATION
Day 1 8:00 am-12:45 pm | Attendees: provider champions, psychiatrist, behavioral health care managers (BHCM), process improvement specialists, practice managers, PO representatives (Providers & psychiatrists will attend the morning session only).
Day 2 8:00 am-12:45 pm | Attendees: behavioral health care managers, supervisors
Guidelines for participation:
- PO and each practice unit completes a practice assessment (one for each practice interested).
- Send completed practice assessment to Sue Vos at email@example.com.
- Schedule a meeting with Sue Vos at firstname.lastname@example.org for the practice and PO representatives to review the assessment.
- Successful Completion of the Base CoCM Training is a prerequisite to attend this training.
- Practice attendees register for each applicable training date.

Attendees of this training include: Family or Pediatric provider champion, psychiatric consultant, Behavioral Health Care Manager (nurse or social worker), and up to three other staff per practice (e.g., clinical supervisor, practice manager, billing specialist, etc.)
FREE for members; FREE for non-members
This training is provided at no cost to the participant through a contract with BCBSM.
Continuing Education Credits
Providers & Nurses
This live series activity, Adolescent Collaborative Care, has been reviewed and is acceptable for credit by the American Academy of Family Physicians. Physicians should claim only the credit commensurate with the extent of their participation in the activity.
Approved for (1 credit per session ) AAFP (Prescribed) credits.
AMA/AAFP Equivalency: AAFP Prescribed credit is accepted by the American Medical Association as equivalent to AMA PRA Category 1 credit(s)™ toward the AMA Physician’s Recognition Award. When applying for the AMA PRA, Prescribed credit earned must be reported as Prescribed, not as Category 1.

This course is approved by the NASW-Michigan Social Work Continuing Education Collaborative
Full continuing education credits are achieved by the successful participation and completion of the entire live training course and all required pre/post-tests and evaluations.
Live training continuing education certificates will be issued by MICMT after the attendee completes the associated posttest and evaluation.
