Specialty Team-Based Care (STBC)
Applying Team-Based Care Principles to Specialty Care Management and Ambulatory Care Professionals
If you are participating in the BCBS specialty pilot initiative, this is the training for you. If you are not a part of this initiative, you should be signing up for the Team-Based Care training.
This training will prepare specialty care managers and ambulatory care professionals in:
- coordinating patient care needs with the primary care team,
- understanding the framework for complex care management roles,
- understanding the elements of integration into the ambulatory care setting, and
- learning the related care management skills.
Michigan Center for Clinical Systems Improvement (Mi-CCSI) has been approved by the Michigan Institute for Care Management and Transformation (MICMT) to provide the MICMT Specialty Team Based Care (STBC) course training.
MICMT requires course completion of all components listed above. BCBSM reimbursement for training fees requires participants to achieve a "Pass" score of 80% or greater on the MICMT STBC Course Statewide Post Test.
RN’s, MSW’s, NP’s, PA’s, and Pharm-D’s who will be working as complex care managers in the ambulatory setting.
Payer Incentives (Blue Cross Blue Shield Of Michigan & Priority Health):
This training is required for care team members who will be providing and billing care management codes under the BCBSM STBC value-based reimbursement incentive.
$190 for members; $245 for non-members
NOTE: Blue Cross Blue Shield Of Michigan has a PDCM training fee reimbursement available for up to $500 within capped agreement with BCBSM. See your physician organization's BCBSM PDCM agreement for more details.
