Post-Acute Continuum Learning Activity
This course is designed as an overview of the post-acute continuum of care and discuss eligibility requirements.
It is intended for care managers, care coordinators and other team members in the primary care or ambulatory care settings. This recorded material is available for nursing and social work enduring continuing education credits.
1. The defined post-acute levels and types of care in the community.
2. Which criteria are used to determine patient eligibility for each level of care in the continuum.
3. The importance of care coordination within the care/case manager role during this process.
