Part Two: Practical application of the pathology and medication in developing a care plan and supporting the patient with self-management actions
Each session is a different topic and is available for on demand viewing and enduring continuing education credits.

From April 15, 2020 through October 15, 2020 to help those who would benefit and who will use the information to benefit others, we will be offering two of our Disease Management on demand online learning series: The seven-part Asthma/COPD series and the upcoming two-part Congestive Heart Failure series at NO COST to the participant.
Part One: Pathology and Pharmacological Management
This session is designed to familiarize the care manager and other care team members with the pathology and pharmacological management of heart failure.
Part Two: Supporting Patient Self-Management & Application to Care Management
This session is designed to familiarize the care manager and other care team members with support strategies for patient self-management and management strategies for the care management team while working with patients with heart failure.
