Session 4: Wellness, Prevention & Use of Registry
This session provides a broad overview of the prevalence of development & risks, diagnosis, key components of medical management, lifestyle changes, self-management promotion and goal setting. It is intended for care coordinators, medical assistants, community health workers, social workers, other members of the primary care or ambulatory care teams, and care managers wanting a refresher on chronic kidney disease (CKD).
- Identify behaviors that help prevent chronic disease
- Review common diseases that may be caused or worsened by personal behaviors
- Review resources for preventive guidelines
- Review preventive care resources for patients
- Review tools to assist the practice with preventive population management

Susan Vos, BSN, CCM, RN
Susan is the director of training programs at the Michigan Center for Clinical Systems Improvement. She graduated with her Bachelor of Science in Nursing degree from Grand Valley State University and is certified in Case Management through CCMC. She has worked in quality, PCMH, and NCQA implementation teams at Spectrum Health Medical Group and West Michigan Physicians Network. She is a former manager of utilization management and case and disease management at Priority Health, an auto/worker’s compensation case manager and kidney transplant coordinator. In addition, she is a Complex Care Management master trainer for the MiCMRC for their MiPCT and SIM initiatives and she is the master trainer for Mi-CCSI’s self-management training and COMPASS (Behavioral Medical Integration) Care Model.
