Basics of Disease Management Webinar Series
The Michigan Center for Clinical Systems Improvement (Mi-CCSI) is hosting a four session webinar series designed to provide introductory level knowledge and to familiarize care coordinators, medical assistants, community health workers and other care team members with the prevalence, diagnosis, risks and key components of medical management for the following chronic diseases:
Session 1: Diabetes
This session provides a broad overview of the prevalence of development & risks, diagnosis, key components of medical management, lifestyle changes, self-management promotion and goal setting.
Session 2: Hypertension, Hyperlipidemia & Obesity
This session provides a broad overview of the prevalence and diagnosis of hypertension, hyperlipidemia & obesity, correlation of the three conditions, importance of treating and managing the three conditions, key components of medical management, lifestyle impact & changes, self-management promotion and goal setting.
Session 3: Chronic Kidney Disease
This session provides a broad overview of the prevalence and diagnosis of chronic kidney disease, diagnosis strategies, key components of medical management, lifestyle impact & changes, self-management promotion and goal setting.
Session 4: Wellness, Prevention, Use of Registry & Data
This session provides a broad overview of the use of electronic registries and utilizing their data, reviewing statistics, reviewing key populations that are serviced, reviewing available services provided by practice and reviewing quality measures.
Each session is a different topic and will be recorded and will be available for on-demand viewing.
Medical Assistants, Health Coaches, Social Workers, Community Health Workers, and other care team members.
Members: No charge (registration is required)
Non-members: $10 per attendee per webinar (registration is required)

Susan Vos, BSN, RN
Susan is the director of training programs at the Michigan Center for Clinical Systems Improvement. She graduated with her Bachelor of Science in Nursing degree from Grand Valley State University and is certified in Case Management through CCMC. She has worked in quality, PCMH, and NCQA implementation teams at Spectrum Health Medical Group and West Michigan Physicians Network. She is a former manager of utilization management and case and disease management at Priority Health, an auto/worker's compensation case manager and kidney transplant coordinator. In addition, she is a Complex Care Management master trainer for the MiCMRC for their MiPCT and SIM initiatives and she is the master trainer for Mi-CCSI's self-management training and COMPASS (Behavioral Medical Integration) Care Model.
