Thank you for registering for our training. The Michigan Center for Clinical Systems Improvement (Mi-CCSI) has been approved by the Michigan Institute for Care Management and Transformation (MICMT) to provide the Introduction to Team-Based Care Training (TBC) course training. This course is designed to prepare care managers, ambulatory care professionals and other members of the primary care practice care team to effectively manage patients in multidisciplinary care team approach.
The Introduction of TBC Course consists of the following components:
- Self-study web based modules (Self-study pre-work is required to be completed prior to attending the live training.)
- Live training, one 8 hour day
- MICMT TBC Course Statewide Post Test and course evaluation
MICMT requires course completion of all components listed above. BCBSM reimbursement for training fees requires participants to achieve a "Pass" score of 80% or greater on the MICMT CCM Course Statewide Post Test. You will also be required to complete a post-training evaluation to receive applicable continuing education credits
