Mi-CCSI partnered with three regional organizations to implement and test the COMPASS model. These partners are Lakeshore Health Network in Muskegon, Mercy Health Physician Partners in Grand Rapids, and Spectrum Health in Grand Rapids. A total of 19 primary care practices in West Michigan participated in West Michigan and cared for over 350 patients.
Mi-CCSI and partner trainers provided comprehensive initial and ongoing training for each practice team implementing COMPASS. Each practice team was comprised of at least a physician champion, nurse care manager, practice manager and other clinical and operational support. The practice team informed other members of the practice and drove implementation. The training included: defining team roles and responsibilities, building effective care manager, physician and team communication, effective patient engagement and communication, managing depression in the primary care setting, managing suicide risk, developing care plans and treat-to-target processes, effective care transitions, patient monitoring and graduation (transition back to routine care), designing implementation strategies, motivational interviewing, and behavioral activation.
