Disease Management: Diabetes Learning Series
Part Two: Team-Based Care Management & Self-Management Support
This session is designed to familiarize the care manager and other care team members with approaches in team-based care management and self-management support for with diabetes mellitus.
This learning activity is intended for care managers and other care team members in the primary care or ambulatory care settings. This learning activity is available for nursing and social work enduring continuing education credits.
- Describe the epidemiology, pathophysiology, and risk factors for diabetes.
- Explain key aspects of the management of diabetes, including medications commonly used for treatment.
COVID-19 continues to impact and challenge us all in one form or another. It is in our coming together as a community, to help and support, that we can help one another and make a difference.

Mi-CCSI’s mission is “partnering to better care” and providing continuing education opportunities at no cost is one way we can contribute at this time.
From October 1, 2020 through March 31, 2021 to assist those providing care, Mi-CCSI is extending and expanding its FREE on-line enduring disease management learning options. All enduring learning activities have nursing and social work continuing education available and are at NO COST to the participant.
Claire Nolan, PharmD
Claire Nolan received her Doctor of Pharmacy degree from the Wayne State University Eugene Applebaum College of Pharmacy and Health Sciences in Detroit, MI. Dr. Nolan has experience in the specialty, ambulatory, and community pharmacy practice settings. She has focused her career on continuous quality improvement, program development, and education.
Robin Schreur, BS, RN
Robin Schreur is a trainer for Mi-CCSI (Michigan Center for Clinical Systems Improvement). Her aim is to instill the importance of patient centered care “one patient story” over the continuum of care. Robin served as the Clinical Lead Care Manager in ambulatory Care for the Spectrum Health system from March 2012 through April of 2020. She has had the privilege of partnering with patients and families in primary care practices as care management has evolved to include behavioral health and primary team care. Through onboarding new staff at Spectrum and teaching at Mi-CCSI, Robin has had the opportunity to train hundreds of clinicians particularly in the Care Management Process and Motivational interviewing.
The presenter and all planners have indicated that they have no commercial interests related to the content of this educational activity.
